I've finished this game now, unlocked all the levels, collected all the coins and spent 'em all cos you get a message informing you that you've spent them. And yet, I only have 2 stars on the file selection screen and not 3.

I only have 2 also.  where did you here that you get 3?  doesnt look like their is enough room for 3

Are you sure you did everything?

As you progress in the game, you will get Stars that are awarded to you for
achieving certain feats.  These goals are:

 i) Save Princess Peach.
ii) Defeat every level in the game, including the Alternate Exits (read
    below for information on which levels have them).
iii) Spend every single Star Coin.

Have you re beat it in challenge mode.  "Not needed for 3 stars"
"Gamers who’ve already finished the game will be able to access this secret Challenge Mode by hitting Start in the world map and pushing the following combination of buttons during the Start Menu"

Puts a scroll lock on it.  Like SMB for nes.  You can't back track in the level.

Oh do you need to win the whole game again in chalenge mode?  Ive done that befor but only won 2 levels then quit

Not sure, haven't completed it myself yet "in challenge mode", and haven't heard of anyone else mentioning completing it in challenge mode.

Not horizontally no, but it appears on top forming a triangle.

The most common thing missed is being fired out of all the cannons.
If your cannon appears as a red dot on the top screen it means you still need to be fired out of it.
In fact none of the location spots should be red (apart from toad houses on the first spot of evey world)

Other obvious things that you've probably completed:
-money says "All * Spent"
-Exit from 7.4 -7.6
-all green pipes open

But I'm guessing its most likely the cannon thing...

Do you have to go through the secret exits in challenge mode as well?

You dont have to do anything in challenge mode to get the three stars...

All the cannons and warp pipes are unlocked. On every world, when looking on the map on the bottom screen, every level is connected and it says that all the coins are spent. I've also gone back and defeated the last boss in Bowser's Castle after spending all the coins. So I'm not sure why I only have 2 stars.

Have you checked the spots under the cannons are blue and not red?
You have to actually use them not just unlock them.
this goes for the mushroom houses too.
Not sure if you have to use the green pipes.

edit 2 - and you definitely dont need to do anything in challege mode as I only found out about that today...
