It's possible the game could be already rated depending on whether the actual content was done. All you have to do to be rated by the ESRB is to send a video of a certain length (like a half hour to an hour) highlighting all the features including any instances of violence, bad language, etc etc.

As long as the actual content of the game hasn't changed between the ESRB rating and release it's perfectly believable they had it rated.

The problem though, was this was released on the eve of april fool's and something this far along would have definitely been highlighted. It is very well done though.
